Working

Apoxet 60mg medication has dapoxetine as its active ingredient. Dapoxetine is a selective serotonin reuptake inhibitor (SSRI) specifically used to treat premature ejaculation (PE) in men aged between 18 and 64. Dapoxetine works by increasing serotonin levels in the nervous system, which can extend the time to ejaculate and improve control over ejaculation.

Apoxet 60mg (dapoxetine) works primarily by influencing serotonin levels in the brain.

Serotonin is a neurotransmitter in the brain that regulates mood and behavior, including sexual function.

Here's how it works to help manage premature ejaculation (PE):

  1. Increases Serotonin Levels: Dapoxetine belongs to a class of drugs called selective serotonin reuptake inhibitors (SSRIs). It increases the serotonin available in the brain by inhibiting its reuptake (reabsorption) into nerve cells. Higher serotonin levels in the brain contribute to better control over ejaculation.
  2. Delays Ejaculation: By increasing serotonin activity, dapoxetine delays the signal for ejaculation, extending the time it takes for a man to ejaculate during sexual activity. It helps prolong sexual performance and can reduce the anxiety associated with premature ejaculation.
  3. Short-acting Mechanism: Unlike traditional SSRIs used for depression, which take weeks to show effects, dapoxetine acts quickly. It reaches peak concentrations within 1 to 3 hours after taking the dose, which is why take it as needed and on-demand before sexual activity. Its effects are temporary, and our body excretes the drug quickly, within a few hours.
  4. Improves Control: Dapoxetine can help men gain better control over their ejaculation reflex, giving them more time and reducing the risk of unintentional, rapid ejaculation.

Dose

Always consult a doctor before deciding on the dose. They will assess the health, the severity of the condition, and any potential risks.

Apoxet (dapoxetine) is available in two main dosages to treat premature ejaculation (PE):

Apoxet 30mg: This is the starting dose recommended for most patients. Take it 1 to 3 hours before sexual activity. The 30mg dose allows the doctor to evaluate the effectiveness and tolerance of the drug before increasing the dose.

Apoxet 60mg: This is the higher dose if the 30mg dose is not sufficiently effective. If the patient tolerates the Apoxet 30mg well but does not experience an adequate improvement in premature ejaculation, the doctor may increase the dose to 60mg.

  • How to Take: Take the medication 1 to 3 hours before the sexual activity. Swallow it with a full glass of water.
  • Frequency: One Apoxet 60mg once in 24 hours.
  • Maximum Dosage: The maximum dose is 60mg per day
  • Starting dose: Most patients will begin with the 30mg dose to assess their response and tolerance.

Precautions

Allergic Reactions

  • Allergy to dapoxetine or any ingredients in the medication: Patients allergic to dapoxetine or any component of Apoxet should avoid using it.

Heart Problems

  • Significant heart conditions: Individuals with severe heart issues, such as
    • Heart failure
    • Significant arrhythmia (irregular heartbeats)
    • Severe coronary artery disease
    • History of fainting because of heart problems. These conditions can increase the risk of complications, such as fainting or sudden drops in blood pressure when taking dapoxetine.

Moderate to Severe Liver Impairment

  • Patients with moderate-to-severe liver problems should avoid using dapoxetine, as your body may not safely process it, leading to increased side effects.

Taking Monoamine Oxidase Inhibitors (MAOIs)

  • Avoid dapoxetine within the last 14 days of using MAOIs for depression and other mental health conditions. Combining dapoxetine with MAOIs can cause serious interactions, including serotonin syndrome, which can be life-threatening.

Concomitant Use of Certain Medications

Dapoxetine can interact with other drugs, potentially causing harmful effects. Contraindicated medications include:

  • Thioridazine (used to treat psychiatric conditions)
  • SSRIs (Selective Serotonin Reuptake Inhibitors), SNRIs (Serotonin-Norepinephrine Reuptake Inhibitors), and other antidepressants
  • St. John's Wort (an herbal supplement for depression)
  • Tricyclic antidepressants or lithium (used for bipolar disorder)
  • CYP3A4 inhibitors such as ketoconazole, itraconazole (antifungals), ritonavir, and saquinavir (used in HIV treatment)
  • Other medications that affect serotonin levels or can cause serotonin syndrome.

Hypotension or Fainting

  • Patients with a history of fainting (syncope) or conditions that cause low blood pressure should avoid using dapoxetine, as it can further lower blood pressure and increase the risk of fainting.

Severe Kidney Impairment

  • Patients with severe kidney problems should avoid taking dapoxetine. Patients with mild-to-moderate kidney impairment may adjust the dosage but exercise caution.

Neurological Conditions

  • Patients with a history of seizures or epilepsy should avoid dapoxetine, as the medication may lower the seizure threshold, increasing the risk of having a seizure.

Use of Alcohol

  • Avoid combining dapoxetine with alcohol because it can increase the risk of side effects such as dizziness, drowsiness, and fainting, leading to potential injuries.

Mental Health Disorders

  • Patients with severe depression, mania, or bipolar disorder should be cautious or avoid using dapoxetine, as it can worsen specific symptoms or interact with antidepressant medications.

Apoxet (dapoxetine) has several drug contraindications when in combination with certain medications because of the potential for serious interactions.

Monoamine Oxidase Inhibitors (MAOIs)

  • Avoid Dapoxetine with MAOIs used to treat depression and some other psychiatric disorders. Combining these drugs can lead to serotonin syndrome, a potentially life-threatening condition characterized by high serotonin levels, leading to symptoms such as agitation, confusion, rapid heart rate, and high blood pressure.
  • Example MAOIs:
    • Phenelzine
    • Tranylcypromine
    • Selegiline
    • Isocarboxazid

Wait for a minimum 14-day gap between stopping MAOIs and starting dapoxetine, and vice versa.

Thioridazine

  • Avoid combining Dapoxetine with thioridazine, an antipsychotic medication used to treat schizophrenia. It may cause significant changes in the electrical activity in the heart, increasing the risk of serious cardiac arrhythmias.
  • Avoid thioridazine during treatment with dapoxetine. Wait for a 14-day clearance period before starting or stopping thioridazine when you have administered dapoxetine.

Other Selective Serotonin Reuptake Inhibitors (SSRIs)

  • Avoid combining Dapoxetine with other SSRIs, which are commonly used for treating depression and anxiety, as this increases the risk of serotonin syndrome.
  • Example SSRIs:
    • Fluoxetine
    • Sertraline
    • Paroxetine
    • Citalopram
    • Escitalopram

Serotonin-Norepinephrine Reuptake Inhibitors (SNRIs)

  • Avoid similar to SSRIs, SNRIs that treat depression and anxiety, and combining them with dapoxetine increases the risk of serotonin syndrome.
  • Example SNRIs:
    • Venlafaxine
    • Duloxetine
    • Desvenlafaxine

Tricyclic Antidepressants (TCAs)

  • Avoid TCAs are another class of antidepressants with dapoxetine because of the risk of increased serotonin levels and serotonin syndrome.
  • Example TCAs:
    • Amitriptyline
    • Nortriptyline
    • Imipramine

Lithium

  • You must not combine lithium used for bipolar disorder with dapoxetine, as it can increase the risk of serotonin syndrome or cause central nervous system (CNS) toxicity.

Tramadol

  • Avoid Tramadol, a pain medication with serotonergic properties with dapoxetine, as it increases the risk of serotonin syndrome.

Linezolid

  • Linezolid is an antibiotic with monoamine oxidase-inhibiting properties. Its combination with dapoxetine increases the risk of serotonin syndrome.;

CYP3A4 Inhibitors

  • Potent CYP3A4 inhibitors can increase the blood levels of dapoxetine, leading to a higher risk of side effects, including fainting and cardiac issues. These drugs inhibit the enzyme responsible for breaking down dapoxetine in the liver.
  • Example strong CYP3A4 inhibitors:
    • Ketoconazole (antifungal)
    • Itraconazole (antifungal)
    • Ritonavir, Saquinavir (used for HIV/AIDS)
    • Clarithromycin (antibiotic)

Use caution when administering dapoxetine with moderate CYP3A4 inhibitors, such as diltiazem and verapamil. You may need dosage adjustments.

PDE-5 Inhibitors (for Erectile Dysfunction)

  • Avoid Phosphodiesterase type 5 (PDE-5) inhibitors, such as sildenafil (Viagra), tadalafil (Cialis), and vardenafil (Levitra). Use them cautiously with dapoxetine. Combining them can increase the risk of low blood pressure (hypotension) and fainting.
  • Use caution while administering this combination.

Alcohol

  • Avoid alcohol with dapoxetine because it can enhance the sedative effects of both substances, leading to increased dizziness, drowsiness, fainting, and impaired judgment.

Other Drugs Affecting Serotonin Levels

  • Avoid medications that can increase serotonin levels, like triptans (for migraines), or opioid analgesics, such as fentanyl and oxycodone. Use them cautiously or avoid them with dapoxetine because of the risk of serotonin syndrome.

Anticoagulants and Antiplatelet

Dapoxetine may affect platelet function, so combining it with anticoagulants (warfarin) or antiplatelet agents (aspirin, clopidogrel) can increase the risk of bleeding. Monitor the effect closely if you use these drugs together.
